Important Update from Mutual Trust Life Insurance Company
Mutual Trust Life Insurance Company recently revised the policy change/reissue applications. So, effective as of March 1st, 2018, the forms split into three specialized forms. As a result, splitting the forms makes policy change/reissue easier for all parties involved. Hence, processing business with Mutual Trust is made easier. Below is the official update from Mutual Trust, including all the details to this update.
We Revised Our Policy Change/Reissue Application
Mutual Trust is excited to announce that they have taken the Policy Change/Reissue Application 6329-12 (including all of its state variations), and they have split it into three new specialized forms to better serve you.
Separate forms are now available for Policy Change, Policy Reissue, and Request for Rider or UL Withdrawal. These forms are available on the Agent Web Site and are effective immediately. Mutual Trust will continue to accept the 6329-12 form as long as it is dated before April 30, 2018.
Form 6333 should be used for policy changes in CA, DE, ND, and SD. State specific versions of the form are available for DC, FL, and MT. There is an ICC version available for use in all other states except for NY.
Similarly, the 6332 form should be used for policy reissues in CA, DE, ND, and SD. This form also has state specific versions for DC, FL, and MT. The ICC version can be used for all other states except for NY.
Any requests for the disbursement of funds from a rider or requests for money from a UL will be handled with the N-007 form. No state specific variations of this form apply.
